Assessment, dressing care and monitoring for complex or ongoing wounds, with communication to your treating team when further review is needed.
02Catheter, stoma & PEG support
Nursing care and practical education for established urinary catheters, stomas and gastrostomy feeding plans. Support is matched to your clinical instructions and individual needs.
03Nursing after a hospital stay
Planned nursing visits to support your transition home, including wound care, clinical observations and follow-through on your discharge plan.
04More complex ongoing care
Discuss referrals involving higher nursing needs, including an established tracheostomy care plan. We assess suitability, equipment, staffing and escalation arrangements before accepting care.
05Education for families & support teams
Practical explanations and care-plan guidance to help the people supporting you understand daily care, changes to watch for and when to seek clinical advice.
Services are subject to individual assessment and nurse availability. NDIS-funded care must relate to your disability support needs and available plan funding.

Do I need a referral?
You can contact us directly to discuss care. Depending on the support required, we may need a discharge summary, treating clinician’s instructions or an existing care plan before visits can begin.
Can my NDIS plan pay for nursing?
Some disability-related nursing supports may be funded through your NDIS plan. Funding depends on your individual circumstances, the support required and how your plan is managed. We confirm arrangements before starting care; we cannot guarantee funding approval.
Can I arrange and pay for care privately?
Yes. You or your family can enquire about self-funded nursing. We discuss the support required, visit schedule, fees and any travel charges before you decide to proceed.
Can you provide urgent or around-the-clock care?
Visits and extended support require advance assessment and confirmed staff availability. We do not provide an emergency response service. For a medical emergency, call 000.
How do I refer a participant or patient?
Call us first to discuss the type of nursing needed, the person’s suburb and the proposed start date. Please obtain their consent before sharing personal information. We will arrange an appropriate way to receive clinical documents.
