Support we can discuss
- Assessment, dressings and monitoring for complex or ongoing wounds.
- Nursing for established catheter, stoma and PEG feeding plans.
- Higher nursing needs, including an established tracheostomy care plan, subject to suitability and staffing.
- Practical care-plan education for family members and support workers.
How funding is considered
The nursing support must relate to your disability needs and available plan funding. We review the support requested and how your plan is managed before confirming whether we can accept the referral.
NDIS funding does not automatically cover every health treatment or nursing visit. Funding and service arrangements need to be confirmed for your situation.
What to share in your first enquiry
Tell us your suburb, the nursing support needed, your preferred start date and whether your plan is self-managed, plan-managed or NDIA-managed. A participant, family member, support coordinator or treating clinician can make the initial enquiry.
From enquiry to agreed care
We discuss your needs, arrange an appropriate way to review relevant clinical documents, and assess staffing and the home environment. Before care starts, we agree on the nursing plan, visit arrangements and fees.
Obtain the participant’s consent before sharing their personal information. Please send only a brief initial enquiry; we will arrange how to receive clinical records.
Where we visit
Western Sydney, South West Sydney and North West Sydney, including the Camden corridor, Penrith and the Hills. Availability depends on your suburb, care needs and the nursing team’s schedule.
