- You are looking for ongoing nursing visits and guidance for the people supporting you.
- You want to discuss whether higher nursing needs can be safely supported at home.
What your care plan includes
We start with a conversation about your needs and review any relevant discharge instructions or existing care plan. We then discuss suitable visits and coordination with your treating team, with your consent.
The schedule is agreed around clinical needs and staff availability. Extended visits require advance assessment and confirmed staffing.
Clear arrangements before booking
Fees depend on the nursing support required, visit length, timing and travel. We explain the proposed arrangements and any travel charges before you decide to proceed. An initial enquiry does not commit you to booking care.

Tell us what nursing support you need, your suburb, when you would like visits to start and whether you have instructions from a hospital or treating clinician.
Our service covers planned home nursing. We do not provide an emergency response; call 000 for a medical emergency.
Where we visit
Western Sydney, South West Sydney and North West Sydney, including the Camden corridor, Penrith and the Hills. Availability depends on your suburb, care needs and the nursing team’s schedule.
